🌿 Addiction Vulnerability Screening Tool

Validated, Culturally Responsive, and DOH-Aligned
In compliance with the Philippine Mental Health Act (RA 11036), DOH Guidelines, and Data Privacy Act (RA 10173).

Purpose: To screen for potential drug use and addiction vulnerability among Filipino adolescents and adults (excluding alcohol). For use in schools, workplaces, clinics, and community settings, under ethical and professional supervision.

Demographic Information

Screening Questions (Past 12 Months)

"Drug abuse" refers to:

  • Using prescription or over-the-counter drugs more than directed
  • Using any drug for non-medical reasons (e.g., to feel high, stay awake, cope with stress)
  • This includes: cannabis/marijuana (buntot kuting), shabu (methamphetamine), tranquilizers, barbiturates, LSD, sagad/codeine, inhalants (rugby/tuslob), bath salts, spice, and others.
Note: This tool does NOT include alcohol or tobacco.

For minors (below 18): Parent/guardian must co-sign.
